TSH is thyroid-stimulating hormone: a messenger that the pituitary, a gland under the brain, sends to the thyroid to say how much thyroid hormone to make. With a high TSH the thyroid is usually running slow, and a low TSH fits an overactive one. Outside pregnancy the usual range at the laboratory that runs the test runs from 0.27 to 4.20 mE/L; in the first trimester the pregnancy hormone hCG pushes the value down and lower limits apply, which is physiology and not an abnormality. Iron is lost with every cycle, and with heavy menstrual bleeding the loss exceeds what diet can replace. Heavy bleeding is the most common cause of iron deficiency in women under fifty. In pregnancy the requirement rises sharply and ferritin falls physiologically in the second and third trimesters, and after the menopause the monthly loss disappears and your store shifts towards the male level.
