Included Markers
1 markersA cycle day 21 progesterone test that may provide insight into whether ovulation has occurred. Progesterone rises after ovulation, and measuring it at the right time in the cycle can offer useful data.
Why this test?
Ovulation is a necessary step for natural conception, yet it can be difficult to confirm without testing. While ovulation predictor kits detect the LH surge before ovulation, a progesterone blood test measures the hormonal response after ovulation has occurred, providing a different type of confirmation.
Who is this test for?
This test may be relevant for women who:
- Want to confirm whether ovulation is occurring in their cycles
- Are trying to conceive and want additional data about their cycle
- Have irregular cycles and want to check for ovulatory activity
- Have been asked by a healthcare provider to measure mid-luteal progesterone
What is tested?
This test measures one biomarker:
- Progesterone: a hormone produced by the corpus luteum after ovulation. A rise in progesterone during the mid-luteal phase may indicate that ovulation has taken place.
What can this test tell you?
A progesterone level that is elevated during the mid-luteal phase (around cycle day 21 in a 28-day cycle) may suggest that ovulation has occurred. Low progesterone at this point may indicate that ovulation did not occur or that the luteal phase response is reduced. The result provides a single data point that can be discussed with a healthcare provider in the context of your overall cycle pattern.
How is the sample collected?
The test requires a blood draw at a certified sample point (afnamepunt). There are over 750 locations across the Netherlands. Timing is particularly relevant for this test, so scheduling your visit around the correct cycle day is advised.
When is this test useful?
This test may be considered when:
- You want to check whether you are ovulating
- You are trying to conceive and want confirmation that ovulation occurs
- Your cycles are irregular and you want to assess luteal function
- A healthcare provider has recommended a day 21 progesterone test
What do the results mean?
Progesterone is shown with your value and the reference range for the luteal phase. Elevated progesterone in the mid-luteal phase may suggest ovulation has occurred. Values that remain low may suggest anovulation. It is worth noting that the timing of the test within the cycle is relevant to interpretation, and a healthcare provider can help place the result in context.
Preparation
This test should be taken on cycle day 21 in a typical 28-day cycle. If your cycle is longer or irregular, the test is best done approximately 7 days after expected ovulation. No fasting is required.
What happens after the results?
Your results are typically available within a few working days. The report shows your progesterone value with the reference range and an explanation. These results can be discussed with your healthcare provider, who can interpret them in the context of your cycle length and pattern.

Monitoring progesterone is important because it may confirm ovulation, help evaluate menstrual cycle health, and support early pregnancy assessment. Abnormal levels could suggest luteal phase deficiency, anovulation, or other reproductive concerns.
