First symptoms of menopause: when does it start and what do you notice?

The first symptoms of menopause often come as a surprise. Many women expect hot flushes and irregular periods, but the earliest signs are often more subtle: sleep changes, mood shifts or a slightly different cycle pattern.

When do the first symptoms start?

Most women notice the first changes between ages 40 and 48. The process is gradual, beginning with small hormonal shifts as oestrogen and progesterone levels start to fluctuate.

Early signs women notice first

Cycle changes are the most recognisable early signal: shorter cycles, heavier or lighter periods, unpredictability. Sleep changes such as lighter sleep and more frequent waking may be caused by falling progesterone. Mood swings, unexplained fatigue and morning joint stiffness are also common early signs.

Later symptoms

As menopause progresses, hot flushes, night sweats, vaginal dryness, reduced libido, concentration problems and weight gain (especially around the abdomen) may develop.

What to do

Keep a symptom diary, consider a hormone blood test (FSH and oestradiol), discuss concerns with your GP, and invest in sleep hygiene, regular exercise and a varied diet.
