Chlamydia IgG Antibody Test

The Chlamydia IgG antibody test detects antibodies produced in response to a past Chlamydia trachomatis infection. A positive result indicates prior exposure but cannot distinguish between an active and a previously cleared infection.

The chlamydia IgG test looks in the blood for antibodies against chlamydia, which the body makes after contact with the bacterium. Normally the result is negative.

  • A chlamydia antibody test cannot tell an earlier infection from one that is still active; for a current infection, a PCR test on urine or a swab is the usual test.
  • A negative chlamydia antibody test does not rule out an infection: not everyone makes measurable antibodies, and the test is less sensitive than a PCR test.
  • In a fertility work-up, a positive chlamydia antibody test (CAT) counts as a risk factor for damage to the fallopian tubes.

Chlamydia IgG: what this test measures

This test measures IgG antibodies against Chlamydia trachomatis in your blood. IgG antibodies are produced by the immune system after exposure to the bacterium and can remain detectable long after the infection has been treated and resolved.

Because IgG antibodies persist in the body, a positive result reflects immune memory rather than current infection status. This test is therefore most useful for understanding past exposure history.

Chlamydia IgG: when is testing worthwhile?

Testing may be considered when investigating unexplained fertility issues, evaluating a history of pelvic inflammatory disease, or assessing past STI exposure. It may also be helpful as part of a comprehensive sexual health evaluation.

Note that for detecting an active chlamydia infection, a PCR-based test is recommended rather than an antibody test.

What raises the value

  • Earlier chlamydia infection

    Chlamydia antibodies can come from an infection that has already cleared or been treated.

  • Current, untreated infection

    A chlamydia infection that is still present can also give a positive result; the chlamydia blood test cannot tell a current infection from an earlier one.

  • Other chlamydia species

    Depending on the test, antibodies against other chlamydia species can cross-react, for example antibodies against the airway bacterium Chlamydia pneumoniae; the RIVM notes much cross-reaction between chlamydia species.

Chlamydia IgG: lifestyle and this value

Practise consistent and correct condom use during sexual contact to reduce the risk of chlamydia and other sexually transmitted infections. Regular STI screening is recommended for sexually active individuals, particularly when changing partners.

Does a positive Chlamydia IgG result mean I currently have chlamydia?

Not necessarily. A positive IgG result indicates that your immune system has encountered Chlamydia trachomatis at some point, but IgG antibodies can remain detectable long after the infection has cleared. A PCR test is needed to determine if you have an active infection.

Can Chlamydia IgG antibodies go away over time?

IgG antibodies may decrease over time but often remain detectable for years or even a lifetime after exposure. A positive result does not necessarily reflect recent infection. Consult your healthcare provider for proper interpretation.

Should my partner also be tested?

If you are concerned about a current infection, both you and your partner may benefit from PCR-based testing for active chlamydia. Open communication about sexual health and testing is an important part of responsible health management.
