Negative pregnancy test but pregnant? Why the strip says no

Negative pregnancy test but pregnant? Yes, that happens, and it is more common than a false positive. A urine test has a threshold.

In the first days after implantation your hCG is often still below it. Test too early or with diluted urine? Then the strip says no while your body is already saying yes.

A blood test has no such threshold. The laboratory measures hCG as a number, even while it is still low. So a negative strip and a positive blood test on the same day can both be correct.

I've read the forum threads on this subject. They are full of women who tested negative three times and were pregnant after all. Those stories are real, and almost all of them share the same explanation: the date.

Can you be pregnant while the test is negative?

Yes, you can be pregnant while the test is negative. Especially in the first days your period is late. The strip only reacts once your urine contains a certain amount of hCG.

Just after implantation that is still very little. It roughly doubles every two to three days. So a few days later the same test can well be positive.

How much hCG a test needs has been measured precisely. Cole and colleagues measured urinary hCG on the day the period was due. To find 95 percent that day, a test had to detect 12.5 mIU/ml (PMID 14749643).

Of the 18 brands tested, one reached that sensitivity. A test that only reacts at 100 mIU/ml found about 16 percent that day. That means such a test stays negative on that day in four out of five pregnant women.

Then there is the urine itself. Do you drink a lot or test in the evening? Then your urine is diluted and the hCG per millilitre lower. First morning urine is therefore the best choice.

How long can you test negative while pregnant?

You can keep testing negative until your hCG rises above your test's threshold. With late implantation that takes longer.

Implantation falls 6 to 12 days after ovulation (PMID 10362823). In 84 percent that is on day 8, 9 or 10. Someone who implants on day 12 is four days behind someone who implants on day 8.

With a doubling every two to three days, that is roughly a factor of three in hCG. That is how one woman tests positive on the first day her period is late. Another only a week later.

The rows come from three sources. Wilcox on implantation (PMID 10362823) and Cole on the threshold of urine tests (PMID 14749643). And Barnhart on the rise of hCG, a median 124 percent per two days (PMID 15229000).

They apply to a 28-day cycle with ovulation around day 14. With a longer or irregular cycle everything shifts.

How often does a negative pregnancy test happen while pregnant?

How often a pregnant woman tests negative depends almost entirely on the day and the brand. On the day of the missed period, it varied widely in Cole's study (PMID 14749643). From 5 percent with the most sensitive test to over 80 percent with an insensitive one. A week later it is rare with almost every brand.

A Dutch figure does not exist. A 2014 review does warn that many brands promise more than they deliver (PMID 25100881). Especially about testing well before the missed period. So do not read the days on the packaging, read the sensitivity in mIU/ml.

Negative at 10 or 20 weeks: the hook effect

A negative urine test far into pregnancy is rare and has its own explanation. At very high hCG values a rapid test can actually stop reacting. That is called the hook effect.

Griffey and colleagues described it in urine tests in an emergency department (PMID 21835572). Pregnant women with high hCG values got a negative strip there.

A laboratory catches this by diluting the blood and measuring again. A strip cannot do that.

If you search for "20 weeks pregnant test negative", this is usually the answer. There is a simpler one too. A pregnancy of 20 weeks does not need a strip to prove it.

What does give certainty?

Certainty comes from a test that can see hCG on the day you do it. That is a sensitive urine test with morning urine from a few days late. Or a blood test that measures the number, however low. Two negative strips are not proof; they are the same threshold twice.

Say you are 5 days late and have two negative strips, both done in the evening. The brand has a 25 mIU/ml threshold. By the table a pregnancy should usually be visible by now.

Diluted evening urine and a late implantation can together still just hide it. A third strip with morning urine, or a blood test, makes the difference here.

When do you see your GP?

See your GP if your period stays away for a few weeks while you keep testing negative. Go sooner with abdominal pain, bleeding or if you feel unwell.

A missed period has more causes than pregnancy. Thuisarts names stress, weight change and the thyroid among them. For those too, a GP is the right place, and a strip is not.

My own rule is: test once, wait two days, test again with morning urine. Does your body still say something different from the strip after that? Then a number is the next thing you want.
