Basophils are the rarest white blood cells. In adults the basophil value sits between 0 and 0.2 x10⁹/l. That equals 0 to 2 percent of the total white blood cell count. These cells contain histamine and heparin. They take part in allergic reactions and in inflammation. A low or absent count is not an abnormality, because the lower limit is zero. This test measures how many basophil granulocytes circulate in your blood. Basophils are one of the five types of white blood cells in the differential.

The result appears in two ways. The first is a percentage of the total white blood cell count. The second is an absolute number in billions of cells per litre. The absolute figure is the more useful one, because a percentage moves with the other cell types.

Basophils are the rarest of all white blood cells. Normally they make up 0 to 2 percent of the total. Converted, that is 0 to 0.2 x10⁹/l in adults.

The basophil is related to the mast cell in tissue. Both contain histamine and heparin. Those substances are released during an allergic reaction.

The basophil granulocyte count is measured automatically within the complete blood count. That panel also counts the red blood cells and the platelets. You therefore rarely request basophils on their own.

One point matters more here than for the other cell types. Automated analysers count basophils least accurately of all. Near the decision limit the measurement uncertainty rises above twenty percent. A striking result is therefore confirmed first with a manual blood smear.

Basophils release histamine and heparin. That involves them in allergic reactions and in inflammatory processes in tissue.

Even so, their number in the bloodstream says less than you might expect. The NVKC notes that basophils act mainly in allergically affected tissue. The count in blood therefore says little about an allergy. A normal value does not rule an allergic reaction out. A raised value does not prove one either.

A value above 0.2 x10⁹/l is called basophilia and is uncommon. The most common causes are reactive in nature. They include prolonged allergic reactions, chronic inflammation and an underactive thyroid. Some autoimmune diseases can also come with a mild rise.

In parasitic infections it is mainly the eosinophil granulocyte count that rises. Basophils may rise alongside, but they are not the best measure of it.

The clinically important point is not about a single measurement. A persistent, unexplained rise is a reason for a doctor to look further. That applies especially when the total white blood cell count is also raised. In that combination a doctor judges whether further investigation is needed.

One mildly raised result is emphatically not a persistent rise. Given the measurement uncertainty of this assay, the same person can produce a different value on repeat. The trend across several measurements therefore weighs more than one number.

A low count has no demonstrable meaning. The lower limit of the normal range is zero, so below normal barely exists here.

Basophils are measured within the complete blood count with differential. You generally do not request them separately, because the value gives little direction on its own.

The test belongs to a general picture of your immune defence and blood production. Within it, basophils sit beside the neutrophils, lymphocytes, monocytes and eosinophils.

A repeat measurement is worthwhile when a rise stands out. The reason is the measurement uncertainty of this assay. A second reading shows whether the rise persists or resolves by itself.

A doctor looks further when a rise persists without a clear explanation. The same applies when other values in the same blood count are abnormal. The combination weighs more than a single figure.

There is no reason to test because of a low or absent count. Zero is a normal result and calls for no follow-up.

No fixed preparation is required. Fasting is not needed for this test. Do mention which medicines you take, because your doctor weighs that when assessing the whole blood count.

A low basophil granulocyte count causes no symptoms. It is not a condition either. The lower limit of the normal range is zero, so a result of 0.0 x10⁹/l falls inside that range. Some healthy adults have no basophils in the counted sample on any given day. On top of that, the count sits near the analyser detection limit. There is no disease that presents on a routine result as a basophil count that is too low. There is therefore nothing to do about it, because nothing is wrong.

A raised basophil granulocyte count causes no symptoms by itself. Any symptoms belong to the underlying cause. With an allergic reaction those may be itching, a skin rash, sneezing or a runny nose. With an underactive thyroid, fatigue, feeling cold and weight gain fit the picture. With chronic inflammation the symptoms vary widely by condition. A mild rise on a single measurement often falls inside the assay measurement uncertainty. That mainly calls for a repeat reading. A rise that persists without explanation should be assessed by a doctor, together with the rest of the blood count.

No lifestyle measure changes the basophil granulocyte count in a targeted way. That is not a shortcoming of this page, but the nature of the value itself.

What you can do is aimed at the cause behind a rise. Limiting known allergic triggers reduces the reaction in tissue. Enough sleep and recovery support a balanced immune system.

An underactive thyroid is one of the reactive causes that come with a mild rise. That is not solved with lifestyle, but with a thyroid test and assessment by a doctor.

Do not take supplements with the aim of lowering this value. No product has been shown to do so, and the value does not call for it.

The most useful thing you can do with this result is see it in proportion. Read the basophil granulocyte count together with the total white blood cell count. Keep your results, so a next measurement can be compared with the previous one. For this assay the trend says more than a single value.

What is a normal basophil value?
In adults the basophil value sits between 0 and 0.2 x10⁹/l. That equals 0 to 2 percent of the total white blood cell count, and 0 to 200 cells per microlitre. These figures come from the NVKC. Laboratories use upper limits from 0.10 to 0.20, so always follow the range printed on your own result.
Why is the basophil granulocyte count given both as a percentage and absolutely?
The percentage shows the share within the white blood cells. The absolute count shows how many cells circulate per litre of blood. The two can diverge: if another cell type rises sharply, the basophil percentage falls without there being fewer basophils. The absolute number is therefore the figure used for interpretation.
How accurately does an analyser count basophils?
Less accurately than for the other cell types. UMC Utrecht publishes a variation of 22 percent around 0.15 x10⁹/l for this assay, rising to 31 percent at lower values. The same healthy sample can therefore land just under or just over the limit on repeat. A striking result is confirmed with a manual blood smear.
Is a basophil value of 0 a problem?
No. Zero is the lower limit of the normal range and therefore a normal result. Basophils are the rarest white blood cells, and a counted volume of blood sometimes contains none. There is no condition that shows up on a routine result as a count that is too low. No follow-up is needed for this.
Which values sit alongside basophils in the blood count?
The differential counts five types of white blood cells: neutrophils, lymphocytes, monocytes, eosinophils and basophils. The complete blood count also holds the total white blood cell count, the red blood cells, the haemoglobin and the platelets. A doctor always reads basophils in relation to those other values.
When is a raised basophil value followed up?
When the rise persists and has no explanation. One mildly raised measurement often falls inside the measurement uncertainty and mainly calls for a repeat. If the rise persists, especially alongside a raised total white blood cell count, a doctor judges whether further investigation is needed.
