Yasmin pill experiences: acne, weight and drospirenone

Yasmin pill experiences come back to two things strikingly often: calmer skin and less bloating. Both trace to the progestogen in this pill, drospirenone. It is not an ordinary progestogen but a relative of spironolactone.

That detail is explained in almost no article, and it is exactly why women find this pill different from their last one. It also explains why a hormone result taken on Yasmin is not your own baseline.

Which hormones are in the Yasmin pill?

Yasmin is a combined pill with 30 micrograms of ethinylestradiol and 3 milligrams of drospirenone. Together the oestrogen and progestogen switch off ovulation. You take it for 21 days then break for seven, during which you get a withdrawal bleed. Yasminelle and Yaz contain the same drospirenone with a lower oestrogen dose.

Drospirenone is derived from spironolactone, a drug used for decades for two properties: it blocks androgen receptors and it acts as a mild diuretic.

Both properties show up in Yasmin. That is where the experiences come from.

Yasmin pill experiences with acne and skin

Many women report calmer skin after three to six months. That fits the anti-androgenic character of drospirenone: less androgenic stimulation of your sebaceous glands usually means less sebum. A systematic review in the European Journal of Obstetrics and Gynecology in 2017 compared Yasmin with standard options for PCOS symptoms, including the skin complaints that go with it.

The first weeks can disappoint. In some women skin gets busier before it turns, and three months is a realistic point before you can say anything sensible about it.

Watch the order of cause and effect. The pill suppresses the hormonal stimulus; it does not treat the underlying cause. Stop, and that stimulus usually returns. What can sit behind hormonal acne is covered in our article on hormonal acne.

Does the Yasmin pill make you gain weight?

With this pill women more often report the opposite: less fluid retention. That comes from the anti-mineralocorticoid effect of drospirenone, which lets your kidneys excrete slightly more sodium and water. On the scales that is usually a few hundred grams to a kilo, and it is fluid, not fat.

A genuine fat-gaining or fat-losing effect of the combined pill has not been convincingly shown in research. What women experience is often the fluid effect, plus the ordinary weight change everyone goes through over a few years.

That same diuretic mechanism has a rarely mentioned flip side: you can retain slightly more potassium. According to the Dutch Farmacotherapeutisch Kompas that warrants caution with kidney problems or alongside potassium-sparing medication. So go through your medication list with your doctor.

Is Yasmin a strong pill?

Strong is not a medical term, but in practice women mean the oestrogen dose. At 30 micrograms of ethinylestradiol Yasmin sits in the usual middle range; Yasminelle and Yaz sit lower at 20. Stronger here does not mean more effective, just slightly more oestrogen per tablet.

Picture two women of 26 with the same jawline acne. One switches from levonorgestrel to drospirenone and sees her skin clear within four months. The other notices nothing and turns out to be dealing mainly with insulin resistance. Same complaint, different engine underneath.

What Yasmin does to your hormone results

This is the part you will not read elsewhere, and it can send a PCOS work-up down the wrong path. Ethinylestradiol makes your liver produce more SHBG, the protein that binds your testosterone. Research in Contraception in 2015 measured that effect with a drospirenone-containing combined pill: SHBG rises substantially and your free testosterone falls accordingly.

In practice that means: measure your androgens while taking Yasmin and you are largely measuring the pill. A normal free testosterone then rules out little, while a raised value stands out all the more.

Side effects of Yasmin

The most reported side effects are tender breasts, headache, nausea, breakthrough bleeding and mood changes, mostly in the first three months. As with any combined pill, the oestrogen is linked to a slightly raised thrombosis risk, which is larger with smoking, excess weight or a family history.

Signals not to wait on: a painful swollen calf, sudden breathlessness, chest pain, or an unusually severe headache with neurological symptoms. Get in touch the same day.

How quickly does the Yasmin pill work?

Start on the first day of your period and you are protected immediately. Start later in your cycle and use extra protection for the first seven days. Skin effects take three to six months.

What is the difference between Yasmin and Yasminelle?

Same progestogen, less oestrogen. Yasminelle contains 20 micrograms of ethinylestradiol against 30 in Yasmin.

Can you skip the break week?

Does acne return when you stop?

Often yes, because the underlying hormonal stimulus returns. How quickly varies a lot between people.

Yasmin owes its reputation to a progestogen that happens to also be an anti-androgen, and that explains both the calmer skin and the reduced bloating. If you want to know what your own hormones are doing rather than what the pill is doing, that is a conversation with your GP about the right moment to measure.
