Provera side effects and what the withdrawal bleed tells you

Provera side effects are usually mild and settle once the course ends. The most reported are bloating, tender breasts, headache, mood swings and nausea. Provera is the brand name for medroxyprogesterone, a synthetic progestogen usually taken for a short stretch, often ten to fourteen days.

In the Netherlands it is not prescribed as contraception. It is used to bring on a period that has stayed away, to calm heavy or irregular bleeding, or as part of hormone therapy. What you notice depends on the dose and on why you were given it.

What does medroxyprogesterone do in your body?

Medroxyprogesterone mimics progesterone, the hormone your ovary makes after ovulation. The uterine lining is converted from its building phase into its mature phase. Stop the tablets and that hormonal support falls away, so the lining sheds. That is the withdrawal bleed.

One important difference from the pill: Provera does not reliably suppress ovulation. You can get pregnant during a course. Anyone who needs contraception needs a different method for it.

Which Provera side effects are most common?

The complaints below appear in the Dutch Farmacotherapeutisch Kompas and in the patient leaflet. For most women they are mild and temporary. How often something happens differs per person and per dose.

Contact your doctor for chest pain, shortness of breath, a painful or swollen leg, sudden severe headache or changes in your vision. These are rare, but they belong in the same day.

Why is the bleed after the course interesting?

This is the part most Dutch pages leave out. A withdrawal bleed carries information. To shed a lining you first need to have built one, and building is what oestrogen does.

Broadly there are two outcomes, and they point in different directions.

This is a pointer, not a diagnosis. The outcome also depends on the dose and on how long you took the tablets. Always discuss the result with the doctor who prescribed the course.

What can you measure around a Provera course?

If your period stays away, the question is usually not whether Provera works, but why your cycle has stalled. A female hormone blood test measures FSH, LH, oestradiol and prolactin, and that combination helps separate the usual causes.

Who should avoid Provera?

There are situations where medroxyprogesterone is not advised. These include a known or suspected pregnancy, unexplained vaginal bleeding that has not yet been investigated, a previous thrombosis or pulmonary embolism, severe liver disease, and breast cancer or a strong suspicion of it.

Tell your doctor which other medicines you take, too. Some epilepsy drugs and certain antibiotics speed up its breakdown, leaving less active substance than intended. The Dutch Farmacotherapeutisch Kompas tracks those interactions, and your pharmacy checks them when dispensing.

Frequently asked questions

Is Provera contraception?

No. Provera tablets do not reliably suppress ovulation and do not protect against pregnancy. Do not confuse it with the contraceptive injection, which uses a much higher dose of a related substance and does work as contraception.

How long after Provera does the bleed come?

Usually within two to seven days of the last tablet. If nothing has happened after two weeks, contact your doctor.

Does Provera make you gain weight?

Many women mainly notice fluid retention and bloating, which fades after the course. A short course is too short to change body weight substantially.

Can you get pregnant while taking Provera?

Yes. Use contraception if you want to avoid pregnancy.
