Lenzetto spray: experiences, how it works and alternatives

A thin layer of spray on the inside of your forearm that dries within a minute, and no patch you can feel through your shirt: for some women that is exactly why Lenzetto appeals. Lenzetto is a transdermal estrogen spray prescribed for menopause symptoms. But discreet does not automatically mean it is the best form for you. What matters is which delivery suits your body and your life.

How does Lenzetto spray work?

Lenzetto releases estradiol through your skin, where it is absorbed directly into your bloodstream. You spray it daily on the inside of your forearm, and the skin absorbs the hormone gradually. This gives relatively stable estrogen levels through the day, without the peaks and troughs you can see with tablets.

Each spray contains 1.53 mg of estradiol. Your doctor can adjust the dose by changing the number of sprays per day (1, 2 or 3). The dose is always set in consultation with your doctor and tailored to your symptoms.

Lenzetto experiences: what do women mention?

Experiences vary, but several points come up often.

What women appreciate:

  • Easy to use and dries quickly
  • No visible patch on the body
  • Reduction of hot flashes in many users
  • Better sleep and mood for some

Points to consider:

  • Skin irritation at the application site in some women
  • It can take 4 to 8 weeks to feel the full effect
  • After applying, the spray should not contact others until the skin is dry

Lenzetto compared to other forms

Which form suits you best depends on your preferences and medical situation. This table lays out the main differences.

FormFrequencySuits
Lenzetto sprayDailyWanting something discreet, fast drying
Patch (Estradot, Systen)1 to 2x per weekNot wanting to think about it daily
Gel (Oestrogel, Sandrena)DailyPreferring rubbing in over spraying
TabletDailyNo skin irritation; note higher thrombosis risk

Our view: when choosing a form, "discreet" is pleasant, but the working principle matters more. All transdermal forms, so spray, patch and gel, bypass the liver and therefore carry a lower risk of blood clots than tablets. For women with a raised thrombosis risk, a transdermal form is often preferred, whether that is a spray or a patch.

Advantages of transdermal delivery

  • Lower thrombosis risk: by bypassing the liver, the risk of blood clots is lower than with tablets
  • More stable levels: fewer peaks and troughs than oral intake
  • Less weight-dependent: skin absorption depends less on your body weight

Safety and what the guidelines say

Many worries surround hormone therapy, often dating from a time when older studies gave a one-sided picture. The Dutch GP guideline (NHG-Standaard De overgang) and the patient information from Thuisarts.nl now paint a more nuanced picture: for most women starting hormone therapy around menopause, the benefits for symptoms and quality of life often outweigh the risks, provided your personal situation is carefully considered. Lenzetto fits into that balance as a transdermal option.

Important points your doctor weighs before you start:

  • Your age and how long ago your last period was
  • A history of thrombosis, breast cancer or cardiovascular disease
  • Whether you still have your uterus (then progesterone is needed)
  • Your own preferences and earlier experiences with hormones

Hormone therapy is therefore not a standard prescription, but tailored care. Lenzetto is a relief for one woman and not the right choice for another. You always make that judgement in consultation with your doctor.

When Lenzetto may not suit you

There are situations where Lenzetto, or estrogen therapy in general, is not prescribed or only after careful consultation. Think of a past thrombosis, certain cancers in your history or unexplained vaginal bleeding. Unexpected bleeding in postmenopause is always investigated first, regardless of any hormone therapy. If you have doubts or symptoms that do not fit, raise them with your doctor rather than adjusting the dose yourself.

Measuring hormones around use

Before starting Lenzetto, a baseline of your estradiol can be useful. After starting, your doctor can monitor your estradiol level to optimise the dose. This always happens in consultation with your doctor. To look more broadly at your hormonal balance, a menopause blood test brings several values into view at once. Frequently asked questions

Do I also need progesterone alongside Lenzetto?

If you still have your uterus, your doctor will almost always prescribe progesterone alongside estrogen to protect the uterine lining. This applies to all forms of estrogen therapy and is decided in consultation with your doctor.

Can I shower after applying Lenzetto?

Wait at least an hour after spraying before wetting the skin, so the hormone is sufficiently absorbed. Otherwise follow the package leaflet.

Is Lenzetto covered by insurance?

In the Netherlands, Lenzetto is usually covered by basic insurance when prescribed by your doctor. Check with your pharmacy or insurer.
