How long implantation bleeding lasts and what is still normal

How long does implantation bleeding last? Usually a few hours to two days. If it lasts longer, that fits a period or another cause better.

That is the short version. The numbers underneath are more nuanced.

What the numbers show

Hasan and colleagues followed 4,539 pregnant women. About a quarter had bleeding in the first trimester.

Most episodes lasted less than three days. That is the best available measure of what is usual.

In that same study, 8 percent of women with bleeding described it as heavy. The rest reported spotting or light bleeding.

Why duration alone says little

There is a reason to be careful with these numbers. Harville and colleagues could see exactly when implantation occurred in 221 women.

Of the 151 who became pregnant, 9 percent had bleeding in the first eight weeks. That bleeding rarely fell on the day of implantation.

The researchers found no support for the hypothesis that implantation causes bleeding. Attributing a duration to implantation is therefore harder than it looks.

When does most bleeding occur?

Here is a detail that is rarely mentioned. In Hasan's study, most bleeding fell between gestational weeks 5 and 8.

Implantation falls around week 3. The peak therefore sits well after it.

That means much of the bleeding people call implantation bleeding happens later than implantation itself.

What to keep track of

If you call, concrete details help more than an impression. Note down four things.

• When it started, counted from your ovulation.
• How much it was, expressed in panty liners per day.
• Whether it increased or decreased.
• Whether there was pain, and on which side.

That third question carries the most weight. Bleeding that is easing behaves differently from bleeding that is building.

What if the bleeding comes back?

Some women see bleeding that stops and returns a few days later. That pattern raises more questions than one continuous episode.

In Hasan's study, 1,207 women recorded 1,656 episodes between them. A woman with bleeding therefore averaged more than one.

Recurring light bleeding is not unusual. On its own it says little about the outcome.

Do keep track of the pattern. If episodes get longer or heavier, that is a reason to call.

An hCG doubling check over 48 hours then gives you more to hold on to than another search.

When to make contact

Thuisarts.nl advises calling the same day for heavy bleeding. The same applies to severe abdominal pain lasting more than two hours, or a fever from 38 degrees.

For dizziness, sweating or black spots in your vision combined with heavy bleeding, call straight away.

According to that same source, roughly 1 in 10 pregnancies ends in miscarriage. Bleeding is therefore not a prediction.

What a blood test adds

If the course is unclear, hCG can help. Barnhart and colleagues established the minimum rise in a viable pregnancy.

It was 24 percent over one day and 53 percent over two days. Two measurements 48 hours apart therefore give direction.

About three weeks, counted from your last period. That feels odd, because fertilisation was only a week or so earlier.

Allow about a week. Your hCG has to rise far enough to become visible in urine.

How much blood is normal at implantation?
A few drops, often only visible when wiping. More than that does not fit the usual picture.

How do I know if I have implantation bleeding?
You cannot tell from the bleeding itself. Only a positive test makes it clear afterwards.
