ESR blood test: what a raised sedimentation rate means

Your ESR measures one thing: how far your red blood cells sink to the bottom of a tube in an hour. The faster they sink, the more likely it is that inflammation is running somewhere. Simple principle, awkward result, because no single number counts as normal for everyone.

That last part is why so many women are needlessly alarmed by their result. Your upper limit depends on your age, and it sits structurally higher in women than in men. A value that is fine for your mother is notable in you, and the other way round.

What is ESR exactly?

ESR stands for erythrocyte sedimentation rate. During inflammation your liver makes more sticky proteins, including fibrinogen. Those proteins make your red blood cells clump together, and clumps sink faster than loose cells. The result comes back in millimetres per hour.

So it is an indirect measure. You are not measuring the inflammation itself, but its effect on your blood cells. That makes ESR slower and less specific than CRP, which measures the inflammatory protein directly and responds within six to eight hours (PMID 12813013).

What is a normal ESR value?

For young women the upper limit usually sits around 20 millimetres per hour, but that limit shifts up as you age. An old rule of thumb from the British Medical Journal calculates the upper limit for women as your age plus ten, divided by two (PMID 6402065). For men it is simply age divided by two.

Imagine two women, one 32 and one 68, both with an ESR of 28 mm/hour. In the younger one that value sits well above her limit and deserves explanation. In the older one it falls within what is expected for her age. Same number, opposite conclusion.

Use this table as a direction, not a law. Every laboratory applies its own reference ranges, and those are printed on your result.

What can a raised sedimentation rate mean?

A raised ESR says something is going on, not what. The value climbs with infection, autoimmune disease, tissue damage, and a number of conditions that have nothing to do with inflammation. That broad range is both the strength and the weakness of this test.

The main causes, and the traps I see most often in women:

That third row is the important one for women. Anaemia lifts your ESR without a trace of inflammation, purely because there are fewer red cells getting in the way. Why does a doctor order ESR and CRP together?

Because they run on different timelines. CRP peaks within a day and falls within days. ESR is slow to start and stays high for weeks, long after you feel better. Side by side they say something about the course, not only the moment.

A high CRP with a normal ESR fits something that has just begun. A normal CRP with a high ESR fits a recovery phase, or a smouldering process such as autoimmune disease. That combination is worked out in the overview on what CRP and ESR mean.

In rheumatoid arthritis, ESR is also used to follow how active the disease is, alongside antibodies like anti-CCP that are far more specific but miss about a third of patients (PMID 17548411). More on that under rheumatoid arthritis symptoms in women.

What do you do with a raised ESR?

One raised value is a snapshot, not a diagnosis. Sedimentation responds to all sorts of things, and a recent infection can keep it up for weeks afterwards. A repeat measurement a few weeks later often says more than the first.

See your GP if your ESR sits clearly above your age limit and you have symptoms with it: persistent fatigue, joint pain, fever or unintended weight loss. Thuisarts and the NHG always look at the whole picture with such a result, never the number alone.

To measure your own sedimentation, you can pick ESR through building your own blood test, preferably together with CRP. I would never order them separately, because the combination is where the information sits.
